The form follows the standard complaint template, and every complaint is filed here. Use email only for attachments. Fields marked * are required. Complaining is free of charge.

We need the power of attorney before we can handle the complaint. Reply to the confirmation email you receive and attach it.

3. The matter

For example the subscription number, confirmation number or the date you subscribed.

Tell us what happened, what you think is wrong, and what you would like us to do. At least 20 characters.

4. Attachments, signature and confirmation

The form does not accept files. You will receive a confirmation email with a reference number. Reply to that email and attach the documents.
